When you wish to remain a member of our Health Insurance Society
 

Insured persons lose their health insurance status automatically the day after retirement. However, those who have been continuously insured for two months or longer prior to the applicable retirement date have the option to maintain health insurance status for two additional years as a voluntarily and continuously insured person. You can receive virtually the same insurance benefits and Health activities as when you were working.

Fill in the necessary details in the form below and submit it to the Health Insurance Society within 20 days after the day you lose your qualification.

* Contact the staff responsible at your establishment or the Health Insurance Society if you need an application form.

* If you complete the application form at the Society office, bring bank account information. (The application form has a space for entering your account information.)




What about the insurance premiums?

The standard remuneration used as the basis for calculating your insurance premiums is the lower of the following: (1) your standard monthly remuneration when you lost your eligibility; (2) the average standard monthly remuneration of all insured persons who are members of the Health Insurance Society at the end of September of the previous year. Health Insurance Society calculates their insurance premiums by multiplying the lesser of the two amounts by the prescribed premium rate. Voluntarily and continuously insured persons pay all of the applicable insurance premiums on their own, but pay no premium for bonuses.
Voluntarily and continuously insured persons also pay full insurance premiums in the event they are aged between 40 and 64.

* As specified in the statutes of the Health Insurance Society, standard remuneration may be calculated based on (1) above, even if (1) is higher than (2).

When are insurance premiums due?

Please pay monthly insurance premiums by the 10th day of each month. You will lose your health insurance status if you fail to pay premiums by the deadline.
An advance-payment option is available. Health Insurance Society offers discounts to those who choose to pay six or twelve months of premiums in advance.



What benefits are available?

In general, you can receive the same insurance benefits for which you were eligible before retirement. However, you will not be eligible for Injury and Sickness Allowance and Maternity Allowance. Be note that in the case of benefit continuing after disqualification, you may continue to receive the above-mentioned benefits as a part of the same allowance benefits after disqualification.



What you lose your qualification?

You will lose your eligibility as a Voluntarily and Continuously Insured Person on the following day (the same day in cases 2 and 5) in any of the following cases:

(1) When the extended insurance period comes to an end
(2) When the persons concerned find employment and enter another medical care insurance
(3) Upon the death of those concerned
(4) When the persons concerned fail to pay their premiums on time
(5) When you become an insured individual covered by the medical service system for persons in the latter stage of old age
(6) If you applied to have your status as a Voluntarily and Continuously Insured Person cleared, the last day of the month including the date on which the application was received
