If you are purchasing and having fitted a corset or other prosthetic equipment on your doctorfs instructions, make the payment yourself. You will receive a refund of the base charge from the Health Insurance Society later.



Procedure  
 

Fill in the necessary details in the form below, and submit it to the Health Insurance Society together with a receipt with a written opinion of an insurance doctor attached.
