Reserving directly operated lodges on the Web
@

You must register through the Web reservation system.

œ How to use

›

You must register as a new user. Have your health insurance card ready before starting registration procedures.

œ Applying for ordinary times (first-come, first-served reservations)

›

Reservations will be accepted starting after midnight in the morning of the first day of the month two months before the month of use.
Make reservations through the Web reservation system. (You do not need to submit an application form after making Web reservations.)

œ Applying during special periods (summer, year-end/New Year’s period, Golden Week) (reservations lottery)

›

Apply for the reservations lottery through the Web reservation system during the period applications are being accepted.
Lottery results will be sent to your registered email address on the date reservations are selected/finalized.

Using this facility

›

The insured person must apply for reservations.

›

This facility is not available for use by a single guest. Each party must consist of at least two persons, including at least one member of the Health Insurance Society (insured person or dependent). Be sure to bring your health insurance card with you. Membership of the Health Insurance Society may be checked on the day of use.

›

In general, reservations may be made for only one unit and for no more than two nights.

›

This facility is not available for use by unaccompanied minors.

›

No meal service is available. To prepare meals yourself, use the utensils and dishes provided in the kitchen. A barbecue oven and portable barbecue stoves are available outside the log houses.

How to apply

›

Reservations will be accepted on a first-come, first-served basis from 9:00 am on the first business day of the month two months before the month that includes the date of use.
Information on how to apply for reservations during special periods will be provided separately.

›

Once a reservation has been accepted, the Health Facilities Department will provide the insured person with an application for use by mail or in person at the department office.

›

Provide the necessary information on the application for use and submit to the Health Facilities Department by mail or at the department office.

›

You will be notified of fees and payment deadlines after the Health Facilities Department reviews the application form. Pay by bank transfer to the account specified by the Health Insurance Society by the payment deadline (eight business days before the date of use).

›

After payment is verified, the Log House Use Receipt/Ticket and Facility Use Information will be provided to the insured person by mail or in person at the Health Facilities Department office.

Since rates vary from facility to facility and by date, check with each hotel or the Laforet Club website.

Overnight guests may use tennis courts, fitness centers, swimming pools, and other facilities (fees may apply).

œ How to apply
1. Make reservations directly by calling the facility or reservations line.

2.

Inform the facility that you are a member of the Tokyo Securities Industry Health Insurance Society.

3.

Provide your desired date(s) of use, number of visitors, name, telephone number, and other information and confirm the reservation.

4.

After confirming the reservation, provide the name of the insured person in whose name the reservation is made, “the name of his or her establishment, contact telephone number, and health insurance card code and number.

5.

Present your (original) health insurance card at the front desk on the day you visit. (If you are eligible for a subsidy, submit the subsidy application form.)

6.

The user is required to pay applicable fees him or herself at the facility front desk.

šSubsidy program for use

A 3,000 yen discount per person per night is provided for insured persons and dependents to use Laforet directly operated facilities (for up to five nights per fiscal year [April 1–March 31]).
Be sure to bring the subsidy application form and (original) health insurance cards for all applicants. These are required for discount eligibility.

The Health Insurance Society will not issue tickets, certificates, or other documents for use.
