How to apply for reservations and notes on applications

Call the contracted facility’s information or reservations center directly to apply for use or make reservations. Be sure to inform the facility that you are a member of the Tokyo Securities Industry Health Insurance Society. Note that the special contracted rates will not apply if you fail to inform the facility that you are a Health Insurance Society member.
(The Health Insurance Society will not issue tickets, certificates, or other documents for use.)
