Procedures for making requests such as those for disclosure of personal information


Society members themselves have the right to demand the disclosure, correction, partial deletion, cessation of use, or removal of, or addition to personal data concerning them.
However, most of the personal data on members held by the Society is held in the forms of notifications and applications required under the Health Insurance Act. Since that law requires compulsory membership in health insurance at each establishment (with the exception of Voluntarily and Continuously Insured Persons), in principle, it cannot be partially deleted or removed based on requests by members.

A request to cease use of personal data held by the Society may result in inability on the part of members to receive insurance benefits or health checkups or to enjoy benefits from other Society health activities.

The Society has established the following rules concerning requests for the disclosure, correction, cessation of use, etc. of the personal data it holds. (No fees apply for such requests.)

Items for which requests may be made

 

Requests may be made concerning the following items. However, please see “3” below concerning detailed medical fee statement and pharmaceutical statement ("receipt").

Requests for notification of purposes of use

Parties who may request disclosure etc.

 

Any of the following parties may request disclosure etc.:

     
 

(1)

The individual covered by the personal information for which disclosure etc. is requested, or his or her surviving family

 

(2)

A legal representative of the individual covered by the personal information for which disclosure etc. is requested, or of his or her surviving family

 

(3)

A representative appointed by the individual covered by the personal information for which disclosure etc. is requested, or appointed by his or her surviving family

Disclosure of receipts.

 

Note the following points concerning disclosure of receipts:

     
 

(1)

Receipts are subject to confirmation by the relevant insurance medical institution that such disclosure would not impede examination or treatment.
Receipts cannot be disclosed if the relevant insurance medical institution does not consent to such disclosure.

 

(2)

The Society cannot respond to inquiries concerning the contents of examination or treatment.

 

(3)

Since disclosure of detailed medical fee statements etc. requires locating the relevant receipts and advance confirmation with the relevant insurance medical institution, this process takes roughly one months.

 

(4)

Receipts are prepared in accordance with certain standards for purposes of billing insurers for the costs of insured medical examination and treatment by the insurance medical institution. Please be aware that they may not include all examinations and treatment not covered by insurance.

 

(5)

Please be aware that if the receipts for which disclosure has been requested cannot be located for any reason, such receipts cannot be disclosed.

4.

Issuing various certificates

 

Issuance of the following certificates may be performed using the traditional procedures for such issuance, instead of the disclosure procedures described above:
