"The Childbirth and Childcare Lump-Sum Grant" is paid when an insured person gives birth and "the Dependents' Childbirth and Childcare Lump-Sum Grant" is paid when a dependent family member gives birth. Don't forget to add the newborn as a dependent in these cases.

When using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions

In addition to the Childbirth and Childcare Lump-sum Grant, our society provides a benefit of our own (additional benefit). You must complete separate procedures to receive these additional benefit, even if you use this system.
If your childbirth expenses are less than the amount of the Childbirth and Childcare Lump-sum Grant paid, you may apply to receive the difference between your actual childbirth costs and the amount of the Childbirth and Childcare Lump-sum Grant paid.

  • * To use this system, conclude a contract with the medical institutions regarding application for the benefits under the Childbirth and Childcare Lump-sum Grant and receipts of it. Please contact the medical institutions where you plan to give birth for details.

  • Copy of the written agreement with the medical care institution
    (Indicating that the system of direct payment will be used and that the insurer to be billed is the Society)
  • Copy of childbirth cost details/billing statement or receipt
    (Indicating the delivery date, number of children born, amount received on behalf of the insured person/dependent, whether the childbirth is covered by the maternity medical care compensation scheme, and confirmation that the details match those on the dedicated claim form)

Applies to: Insured persons and dependents using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions

Notes: Even if childbirth expenses are less than the amount of the Childbirth and Childcare Lump-Sum Grant to be paid, the amount paid will not change.

When not using a system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions

Fill in the necessary details in the form below, get a doctor or midwife to certify that a birth took place, and submit it to the Health Insurance Society.

  • Copy of the written agreement with the medical care institution
    (Indicating that the system of direct payment will be used and that the insurer to be billed is the Society)
  • Either of the following:
    • Copy of childbirth cost details/billing statement
      (Not indicating the amount received on your behalf, but indicating whether the childbirth is covered by the maternity medical care compensation scheme)
    • Copy of receipt
      (If the childbirth is covered by the maternity medical care compensation scheme, indicating the fact thereof)
Deadline: As soon as possible
Applies to: Insured persons or dependents who have paid the entire amount of childbirth expense at the medical care institution without using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf

Notes:

The following documents must be attached for a childbirth overseas:

  • Documents issued by the overseas medical care institution or other facility where the birth took place (birth certificate, receipts, etc.)
  • Birth certificate issued locally (original)
  • Translation of the birth certificate
  • A copy of document(s) attesting to the fact of travel overseas (passport, etc.)
  • A letter of consent permitting inquiries with the overseas medical care institution or other facility where the birth took place regarding the fact of the birth, details, and so forth

Add the child

After the child is born, take the necessary steps to add him or her as a dependent.
