Health insurance cards are no longer newly issued or reissued since December 2, 2024. Use your My Number Card as your Health Insurance Certificate.

If you lose your Health Insurance Eligibility Certificate

Required documents:	For persons still employed who do not have a Myna health insurance card Application Form for Issue/Reissue of Health Insurance Eligibility Certificate
Required documents:	For retired persons and those who do not need a Health Insurance Eligibility Certificate Notification of Unrecoverable Health Insurance Card (Elderly Benefits Card/Health Insurance Eligibility Certificate)
Applies to:	Insured persons and dependents who wish to have a Health Insurance Eligibility Certificate issued (reissued) for the following reasons: Lost Individual Number Card In the process of renewing Individual Number Card Individual Number Card electronic certificate has expired Individual Number Card issued but not registered for use as a health insurance card No Individual Number Card issued Individual Number Card returned Support from a third party (e.g., caregiver) required to receive examinations and treatment with a My Number Card as a Health Insurance Certificate Lost or damaged Health Insurance Eligibility Certificate
