Family health examination at another medical care institution

How to apply for and get an examination

  • (1) Choose your desired medical care institution, consent to the Health Insurance Society’s policies on the Handling of Personal Information, and apply to the institution directly. You do not need to notify the Health Insurance Society after this.
  • (2) Pay all examination costs to the medical care institution on the date of the examination. Be sure to obtain a receipt.

Applying for the subsidy

  • (1) After you have received the examination results, etc., submit the following documents to the Health Insurance Society (Health Management Department):
    • Subsidization Application (for individual applications)
    • Receipt (original) (clearly indicating a description of the charge such as “Health examination charge,” the examinee’s name, and the date of the examination)
    • Copy of the notice of health examination results (copy of the page showing the date of the examination, the examinee’s name, and the results)
    • If the examinee is aged 40 or older, the standard questionnaire or equivalent questionnaire
  • (2) The Health Insurance Society will pay the subsidy after reviewing and making a decision on the application.
  • (3) Note that applications are accepted only during the fiscal year of the examination.
    * The subsidy may be paid through bank transfer. If you request payment by bank transfer, payment may be made only to the account in the name of the insured person.
  • Available for persons aged 40 and older only
  • (4) If you are eligible for specific health guidance (motivational support, proactive support) based on the examination results, the Health Insurance Society will send you a notice of specific health guidance.
  • (5) Bring documentation verifying your health insurance eligibility with you to the providing facility to receive specific health guidance.
