How to apply for and get an examination
- (1) Provide the necessary information on the family health examination application and affix your seal, then send to the Health Management Department by post or fax.
* The application form is the same one used for family health examinations at the examination center on the second floor of Kenpokaikan. - (2) Information on the above facilities and the appointment slip/questionnaire will be mailed based on the application.
- (3) Choose your desired facility and make an appointment directly by telephone.
- (4) On the examination date, be sure to consent to the Society’s Handling of Personal Information and bring ① documentation verifying your health insurance eligibility and ② the appointment ticket/medical questionnaire with you to the examining medical care institution. You cannot get examined if you do not provide your consent or if you fail to bring documentation verifying your health insurance eligibility and the appointment ticket/medical questionnaire with you.
- (5) Pay the examination cost of 2,000 yen at the examining medical care institution, then receive your examination.
