• Enter the insurer number and insured person/dependent code and number and then register member information.

  ②Health insurance eligibility information
  ③Notice of eligibility information
  ④Health Insurance Eligibility Certificate
  Reference: Checking health insurance eligibility information (insurer number and insured person/dependent code and number)
• A registration completion email will be sent to your registered email address.
  Enter the login ID and password provided in the email to use Club Off One.
• Choose the category you want to use.
