②Health insurance eligibility information
③Notice of eligibility information
④Health Insurance Eligibility Certificate

* Former Society insured persons may not apply to use the Yamanaka Lake Log Houses. They may only apply to use directly operated lodges.

Reservations can be made via the reservation lottery or on a first-come first-served basis.

[Lottery application]
Lottery applications are accepted during the lottery application acceptance period (in principle, from the first through the seventh of the month two months before the month of use) from “Lottery Application” on the Recreation Facility Web Reservation System. Lottery results will be sent to your registered email address on the lottery date (in principle, the ninth of the month two months before the month of use).
- * You may apply for a lottery for up to two consecutive nights at only one Yamanaka Lake Log House.
[First-come, first-served reservations]
First-come, first-served reservations are accepted from 12:00 a.m. on the day after the lottery date through five business days before the date of use from “First-Come, First-Served Reservations” on the Recreation Facility Web Reservation System.
