Handling of personal information in health checkup services

The Tokyo Securities Industry Health Insurance Society ("the Health Insurance Society" hereinafter) handles personal information obtained through the management of its health checkup services as outlined below. Please review the following before applying for these services.

  • Personal information obtained
    • A. Patient health insurance card code, no., and ID no.
    • B. Patient name, date of birth, gender, and age
    • C. Patient place of employment, contact telephone no., and email address (when using the appointment system at the examination center on the second floor of Kenpokaikan)
    • D. Type of checkups
    • E. Name of medical institution performing the checkups, responses to questions asked during checkup interviews, checkup data, and checkup results
    • F. Other information deemed necessary by the Society
  • Purposes of use of personal information in health checkup services
    • A. To confirm patient eligibility and manage appointments and checkups
    • B. For health consultations and education and health guidance
    • C. To confirm details of health checkups such as checkup items for payments made by the Health Insurance Society to medical institutions performing health checkups
    • D. To confirm details of health checkups such as checkup items for payments made by the Health Insurance Society to entrepreneurs as subsidies for health checkups
    • E. Analysis of checkup results
    • F. Special medical checkups and special health guidance services
    • G. For joint use of results of examination items with the employer
    • H. Other purposes related to Health Insurance Society operations
  • While personal information is obtained and used within the scope outlined above, it may also be obtained and used beyond this scope in the following cases:
    • A. If required by law
    • B. When needed to protect the life, health, property, or other important interests of the individual in question
    • C. Cases in addition to those described under A and B above wherein the Society has deemed there to be appropriate reason for collection of information on the individual concerned from a third party
  • Elective nature of provision of personal information
    While the provision of your personal information to the Health Insurance Society is completely voluntary, failure to provide personal information may result in ineligibility for the health checkups subsidies provided by the Society.
  • Measures to safeguard personal information
    The Health Insurance Society implements various safeguards, including control of access to locations where personal information is stored (including servers), password controls, and access controls, to prevent unauthorized access to or the loss, damage, leakage, or unauthorized alteration of such information.
  • Rights of the individual in question to demand disclosure, etc. of personal information
    The individuals in question have the right to demand the disclosure, correction, deletion, or cessation of use of their personal information obtained by the Society.
  • Subcontracting health checkup operations
    To facilitate health checkup activities, operations such as providing health checkups, the checkup appointment system, certain examination operations, health checkup data entry, specific health checkups and specific health guidance, and retention, transport, and disposal of data and documents are subcontracted.
