Handling of personal information of lodge applicants and users and registered users of the Web reservation system
The Tokyo Securities Industry Health Insurance Society (“Society”) handles personal information of lodge applicants and users and registered users of the Web reservation system (“user in question”) as outlined below.
- Personalinformation obtained
- A. Health insurance card code and no., name, gender, relationship, date of birth, age, address, contact telephone no., email address, and name and telephone no. of place of employment of the user in question
- B. Contact telephone no. of representative user
- Purposes of use
- A. To provide appropriate services to the user in question
- B. To provide information needed by the user in question
- C. To carry out operations, administration, and management of lodge facilities appropriately
- D. To handle legal and administrative procedures
- E. To bill for lodge facility usage charges
- Provision of personal information to third parties
The user’s personal information will not be provided to external parties without the advance consent of the user in question. However, personal information may be provided to third parties in the following cases:- A. As required by laws and regulations
- B. When necessary to protect human life, health, or property in cases in which it would be difficult to obtain the consent of the user in question
- C. When there are reasonable grounds for provision of personal information to third parties in cases other than those under A and B above
- Security measures to protect personal information
The Society implements security measures, including control of access to storage locations (including servers), password protection, and access restriction, to prevent unauthorized access to or the loss, destruction, alteration, or leakage of personal information. - Subcontracting operations
In operating lodge facilities, certain operations are subcontracted. Subcontractors are supervised regarding protection of personal information under subcontracting agreements. The main operations subcontracted are as follows:
Administration of lodge facilities, administration of data and other information concerning the user in question, retention and disposal of documents, etc. - Rights of the user in question
The user in question has the right to demand the disclosure, correction, deletion, cessation of use, etc. of personal information managed by the Society.
