How to apply for and get an examination

  • (1) Apply by completing the necessary information on the Family Health Examination Application and sending it to the Health Insurance Society (Health Management Department) by post or fax.
  • (2) Based on the application, the Health Insurance Society will send a questionnaire form and other documents to your home address. When you receive these documents, be sure to make an appointment for the examination date via the Health Examination Appointment System (website) below.

  • (3) Consent to the Health Insurance Society’s policies on the Handling of Personal Information, enter the necessary information on the questionnaire form sent to you in advance and go directly to the Kenpokaikan on the examination date. Before doing so, stop by the Health Management Department on the fifth floor to pay the examination cost of 2,000 yen.
  • (4) Examination results will be sent to your home address around the Friday two weeks after the examination date.
  • Only those aged 40 and older
  • (5) If you are eligible for specific health guidance (e.g., motivational support, proactive support) based on the examination results, the Health Insurance Society will send you a notice of specific health guidance.
  • (6) Bring documentation verifying your health insurance eligibility with you to the providing facility to receive specific health guidance.
