Family health examination at a contracted medical care institution

(1) Apply directly to your desired contracted medical care institution for an examination. Inform the institution that you are applying for a family health examination under the Tokyo Securities Industry Health Insurance Society and provide information such as your health insurance code and number, name, gender, and date of birth. You do not need to notify the Health Insurance Society after this. Consent to and sign the Health Insurance Society’s document Handling of Personal Information and submit to the medical care institution before the examination. (Consent to Handling of Personal Information is required to undergo an examination.)
(2) Pay any amount beyond the subsidy amount of 7,000 yen on the date of the examination.
Only those aged 40 and older
(3) If you are eligible for specific health guidance (e.g., motivational support, proactive support) based on the examination results, the Health Insurance Society will send you a notice of specific health guidance.
(4) Bring documentation verifying your health insurance eligibility with you to the providing facility to receive specific health guidance.
