Specific health checkup at a group contracted facility (dependents)

Since December 2, 2025, health insurance eligibility is verified using one of the following:

  • ①My Number Card as your Health Insurance Certificate
  • ②Eligibility Verification Certificate
  • ③My Number Card and Eligibility information screen on Mynaportal
  • ④My Number Card and Notice of Eligibility Information

Procedures

* A checkup ticket sent from the Health Insurance Society is required.

Eligible persons

Dependents aged 40-74 years who wish to get a specific health checkup only
Health examinations provided as part of the Society’s health activities may be undergone only once per fiscal year (April 1 through March 31 of the following year).
Note that those who have undergone or plan to undergo the examination under ① - ③ below are not eligible. If you undergo the examination more than once, you will be billed later for the full cost of any subsequent examination.

  • ①Examinations at the examination center on the second floor of Kenpokaikan
  • ②Health examinations at contracted medical care institutions
  • ③Health examinations at other medical care institutions
  • ④Complete medical checkup

Examination period

June 1 through March 31 of the following year

NFHIS group contracted facility
