How to apply for and get an examination

• （1）Check the name, expiration date, and other information on the checkup ticket sent from the Health Insurance Society.
• （2）Choose from the list of NFHIS group contracted facilities to get the examination and make an appointment directly. Be sure to inform the facility that you would like to undergo a specific health checkup.
• （3）On the examination date, bring your checkup ticket and documentation verifying your health insurance eligibility to the examination.
• （4）If you are eligible for specific health guidance (motivational support, proactive support) based on the examination results, the Health Insurance Society will send you a notice of specific health guidance. Bring documentation verifying your health insurance eligibility with you to the providing facility to receive specific health guidance. (No copayment applies.)
