Complete medical checkups at contracted medical care institutions (Insured persons)
Consent to and sign the Health Insurance Society’s document Handling of Personal Information and submit to the medical care institution before the checkup. (Consent to Handling of Personal Information is required to undergo a checkup.)
- * Voluntarily and Continuously Insured Persons and their dependents still need to submit the Complete Medical Checkup Application in advance (at least once week before the planned date of the checkup).
Flow of steps from application through billing (excluding for Voluntarily and Continuously Insured Persons and their dependents)
- ①Examinee makes appointment with contracted medical care institution.
* Must notify his or her establishment of the planned checkup.
- ②The contracted medical care institution sends checkup information, the examination kit, etc. to the examinee.
- ③Consent to the Health Insurance Society’s document Handling of Personal Information and undergo the complete medical checkup.
* Present documentation verifying your health insurance eligibility when submitting the letter of consent. - ④The contracted medical care institution sends the bill, results, etc. to the Health Insurance Society.
- ⑤The Health Insurance Society pays the contracted medical care institution.
- ⑥⑦The Health Insurance Society bills the examinee for the copayment, through the establishment.
- ⑧⑨The examinee pays the copayment, through the establishment.
