How to apply for and undergo health examinations (for establishments)

Establishments

At the end of each fiscal year, establishments will be surveyed regarding whether they request health examinations to take place at the Examination Center on the second floor of Kenpokaikan.
A detailed notice will be sent at a later date to the establishments that request health examinations at the Examination Center.
Check the details and then submit applications collectively on an establishment basis.
Upper Digestive Tract Appointment Examination Tickets will be sent to the establishment based on the application.
Distribute the Upper Digestive Tract Examination Appointment Tickets to the insured persons.
Insured persons

When you receive the Upper Digestive Tract Examination Appointment Ticket, choose the facility you wish to use from the examining medical care institutions shown on the Upper Digestive Tract (Stomach) Examination Information and make an appointment with the facility directly.

  • * You must make an appointment separately from the health examination. Note that Web appointments are not available.
  • * When making an appointment, provide your insured person/dependent code and number and relationship (insured person or dependent).
Complete in advance the questionnaire form on the right side of the Upper Digestive Tract Examination Appointment Ticket and consent to “Handling of Personal Information” before undergoing the examination.
Establishments

The examining medical care institution will send the result report to the individual examinee.

  • * Note that the result reports will be sent to the establishment for examinations received at the Examination Center on the second floor of Tosho Kenpokaikan and Soiken.

Additional applications are accepted at any time during the fiscal year.
Fill out the Health Examination Addition (Change/Cancellation) Application Form and send it by fax or mail.

  • * Applications for planned new employees should be submitted after obtaining Society eligibility.

Be sure to bring ① documentation verifying your health insurance eligibility and ② the appointment ticket/medical questionnaire with you to the examining medical care institution.
Failure to bring these may result in ineligibility for the examination or an additional cost paid at the medical care institution.
