Influenza vaccinations
Influenza vaccinations are available through the following:
- Vaccination at medical care institution contracted to Toshinkyo (no subsidy application to the Health Insurance Society required)
- Vaccination at an ordinary medical care institution (subsidy application to the Health Insurance Society required)
- Vaccination in the examination center on the second floor of Kenpokaikan (no subsidy application to the Health Insurance Society required)
- * About Toshinkyo
Toshinkyo, officially the Tokyo General Cooperative Health Facilities Promotion Association, is a corporation established to promote and raise awareness of the health facility activities operated by general health insurance societies located in Tokyo, jointly implement efficient health facility activities by health insurance societies, and share activities among health insurance societies.
1. Vaccination at medical care institution contracted to Toshinkyo (no subsidy application to the Health Insurance Society required)
Procedural flow through vaccination
①Make an appointment to be vaccinated.
Choose your preferred vaccinating medical care institution at the Toshinkyo website and call the institution directly to make an appointment. Inform the institution that you would like to use a Toshinkyo vaccination ticket. (If you fail to inform the institution, you may be unable to receive the subsidy.) You do not need to contact the Health Insurance Society after this.
②Obtaining your vaccination ticket
On the Toshinkyo website, choose the medical care institution or location where you made your appointment, enter the necessary information, and print the ticket.
You must enter the insurer no. to obtain a ticket. Have documentation verifying your health insurance eligibility ready to obtain the vaccination ticket. [Health Insurance Society’s insurer no.: 06131932]
③On the date of vaccination
Present your vaccination ticket with the necessary information filled out and documentation verifying your health insurance eligibility at the medical care institution and receive the vaccination.
Precautions
- Note that you will not be able to receive the vaccination if you forget to bring documentation verifying your health insurance eligibility.
- Ages at which children are eligible to be vaccinated at medical care institution vary by institution. Check with the institution for more information.
- You cannot apply to the Health Insurance Society for a subsidy for any copayment you paid beyond the maximum subsidy amount when using Toshinkyo.
- One ticket per person must be submitted for each vaccination. If you plan to use multiple tickets, make copies in advance.
- Contact the Health Management Department of the Health Insurance Society if you do not have a printer or if you have any other questions.

2. Vaccination at an ordinary medical care institution (not contracted to Toshinkyo) (subsidy application to the Health Insurance Society required)
Application procedures
(1) Individual applications:
When the insured person or dependent has paid the entire amount of the cost to the medical care institution
- Check the list of vaccinating medical care institutions on the Toshinkyo website to confirm that your preferred medical care institution does not administer vaccinations under contract to Toshinkyo. (Vaccination fees may vary depending on whether or not a Toshinkyo program applies.)
- Receive the vaccination at your preferred medical care institution and obtain a receipt from the institution. When doing so, make sure that the receipt clearly indicates the date of vaccination, the name of the person vaccinated, and a description of the charge such as “Influenza vaccination charge.”
Note that a detailed medical statement or influenza vaccination certificate is not a receipt. Check at the counter when paying the medical care institution to make sure you receive a proper receipt.- Note:If two or more people are vaccinated at the same time, obtain a separate receipt for each. (If the medical care institution is unable to issue separate receipts, ask it to indicate on the receipt a breakdown showing the name and charge for each person vaccinated, at a minimum.)
- Complete and affix your seal to the Application for vaccination subsidy (reverse side: Vaccination recipient details), attach the original receipt, and submit these to the Health Insurance Society (Health Management Department). Note that an influenza vaccination certificate or detailed medical statement cannot be attached in place of a receipt. Be sure to attach the original receipt.
- * For the applicant name, enter the name of the insured person.
- * Only one application will be accepted per person per fiscal year. When applying for a child under 13 years of age who received two vaccinations, wait to apply until after the second vaccination is complete. If the vaccinations were received at more than one medical care institution, indicate this under “Name of medical care institution” (for example, by entering “_____ Hospital and other institution”).
After the second vaccination, apply once for the entire household. (When receiving two vaccinations, they will not be eligible for the subsidy if applied for separately.)

(2) Establishment application:
When applying together for the entire establishment (branch or office)
- Complete the Application for vaccination subsidy (reverse side: Vaccination recipient details), attach the original receipt, and submit these to the Health Insurance Society (Health Management Department). (For the applicant name enter the name of the employer or the health manager, and for the transfer account enter an account in the establishment’s name.)
- If the number of people vaccinated is large, an Excel spreadsheet or similar document may be prepared instead of using the Vaccination recipient details on the reverse side of the form, using a format similar to that of the form.
- When also applying for dependents, apply for the entire household together. Note that additional applications will not be accepted after you have applied once.

How to get vaccinated
Go directly to the vaccination location. Be sure to bring with you the vaccination medical interview sheet (distributed in advance inserted in Kenpo News and posted under “News & Topics” on the Health Insurance Society website) together with documentation verifying your health insurance eligibility (for confirmation of eligibility and reception) with you. No advance reservations are required.
