A medical care certificate for the elderly is issued
 

Patient cost-sharings for persons aged 70-74 are either 20% or 30%, depending on their income. In order to make it easier to identify each person's payment level, the elderly person and each dependent is issued a medical care certificate for the elderly.(In general, presenting an elderly benefits card will ensure that the amount you pay at a hospital will not exceed the cost-sharing maximum amounts. In certain cases, you may also need to present a Certificate of Application of Maximum Copayment Amount.)

When you are treated at a medical institution, please present both your "medical care certificate for the elderly" and your insurance card.

Note that when your cost-sharing rate changes, you also need to change your medical care certificate for the elderly.

* Active workers II or active workers I need to submit a Certificate of Application of Maximum Copayment Amount to avoid paying more than the cost-sharing maximum amount at a hospital.
If you expect to incur high medical care costs, apply to the Society to obtain a certificate in advance.
