Tokyo Securities Industry Health Insurance Society
 

The Tokyo Securities Industry Health Insurance Society (the "Society") engages in the following businesses: the provision of insurance benefits, including medical care expenses for sickness and injury and allowances for sickness and injury and maternity; the execution of health activities designed for health maintenance and promotion, including providing health education, offering health consultations, conducting medical checkups and managing recuperation facilities; and the healthcare provided by clinics.
In order to promote smooth operations of these businesses, personal information of the insured persons and their dependents (including former dependents) is essential. Therefore, in recognition of the necessity for addressing as important challenges the secure handling and management of personal information, all officials and staff members and related parties of the Society hereby declare that they actively pursue and make efforts to promote the protection of personal information.

In addition to complying with all laws, regulations, and other rules concerning the protection of personal information, the Society shall establish a system for the protection and management of personal information and internal rules on the collection, use, and provision of personal information, and shall conduct all relevant operations appropriately in accordance with this system and these rules.

The Society shall respect the rights of individuals concerning personal information and shall handle?appropriately and pursuant to all applicable laws and regulations (such as the Health Insurance Law), as well as in accordance with its own internal rules?any related inquiries from the individuals concerned, as well as any requests from such individuals to disclose, correct, delete, or cease use of such personal information.

The Society shall consistently endeavor to ensure the security and accuracy of personal information and to protect such information, by exercising proper control in the following ways:

 

(1)

Making responsibilities clear by appointing persons responsible for the protection and control of personal information

 

(2)

Implementing measures to prevent unauthorized access to personal information and to prevent the loss, damage, destruction, falsification, leakage, misuse, or similar inappropriate use of personal information, and promptly implementing corrective measures in the event of any problem relating to personal information

 

(3)

Thorough training of employees on the appropriate handling of personal information

In collecting personal information, the Society shall make clear to the member concerned the purpose of such collection of information, except where such collection is required under laws and regulations such as the Health Insurance Law. The Society shall use the personal information collected only within the range of purposes for which such information was collected and shall not provide personal information to any third parties, except in cases such as when subcontracting operations to a third party in order to achieve the purposes for which the information was collected.
When a Society clinic collects personal information on its own as required for checkup, treatment, and other medical services, the clinic shall use such personal information only as necessary for medical purposes (such as checkups). Such personal information shall be used only within the range of purposes for which the information was collected and shall not be provided to any third parties, except in cases such as when subcontracting checkup or other operations to a third party and when such disclosure is required under the relevant laws or regulations.
In addition, the Society shall obtain the advance consent of the individual concerned before using personal information for any purpose outside the initial range of purposes for which such information was obtained.

5.

When subcontracting operations to a third party in order to achieve the purposes for which personal information was collected, the Society shall employ appropriate controls and supervision with regard to the third party concerning the handling of personal information.

6.

This Fundamental Policy and the Society’s system for and internal rules on the protection and management of personal information shall be revised as appropriate in response to the establishment, amendment, or abolition of relevant laws and regulations or other changes in circumstances and whenever a change is considered necessary as a result of periodic audits, to ensure that the Policy, system, and rules may be continually improved.
