When you join the National Health Insurance
 

National Health Insurance covers a range of community residents, including agricultural workers, the self-employed, freelancers, and so forth. It is managed jointly with prefectures and municipalities.
The benefits provided by National Health Insurance are similar to those provided by health insurance societies: in general, insurance benefits are paid at 70% of medical care costs. Copayments for high-cost medical care and inpatient care are handled similarly to those handled by health insurance societies.
National Health Insurance premiums (taxes) vary from municipality to municipality. Check with your municipality for detailed information.
