In addition to complying with
all laws, regulations, and other rules concerning the protection
of personal information, the Society shall establish a
system for the protection and management of personal information
and internal rules on the collection, use, and provision
of personal information, and shall conduct all relevant
operations appropriately in accordance with this system
and these rules. |

In obtaining personal information, except when obtaining such information is required by the Health Insurance Act or other laws or regulations, the Society shall clearly indicate the purposes of obtaining the information to the individuals in question; use the personal information obtained only within the scope of the purposes of use; and refrain from handling personal information beyond the scope necessary.
Society clinics shall obtain personal information within the scope necessary for examinations, treatment, and other operations; use the information obtained only within the scope of the purposes of use; and refrain from handling personal information beyond the scope necessary.
The consent of the individual in question shall be obtained before using personal information beyond the scope of the initial purposes of use.

The Society shall respect the rights of individuals concerning personal information and shall handle this information appropriately pursuant to all applicable laws and regulations including the Health Insurance Act and in accordance with its own internal rules, when it answers any related inquiries from the individuals in question, as well as any requests from such individuals to disclose, correct, delete, or cease use of such personal information.

The Society shall consistently
endeavor to ensure the security and accuracy of personal
information and to protect such information, by exercising
proper control in the following ways: |

Making responsibilities clear
by appointing persons responsible for the protection and
control of personal information |
|
(2) |
Thorough training of employees on the appropriate
handling of personal information |
|
(3) |
Properly managing and supervising subcontractors in their handling of personal information when subcontracting operations to achieve the purposes of use of personal information |
|
(4) |
Implementing measures to prevent unauthorized access to personal information and to prevent the leakage, loss, damage, or misuse of personal information; promptly implementing corrective measures in the event of problems related to personal information |

5. |
This Fundamental Policy and
the Society’s system for and internal rules on the protection
and management of personal information shall be revised
as appropriate in response to the establishment, amendment,
or abolition of relevant laws and regulations or other
changes in circumstances and whenever a change is considered
necessary as a result of periodic audits, to ensure that
the Policy, system, and rules may be continually improved. |
