The Tokyo Securities Industry Health Insurance Society (“Society”) handles personal information of applicants and users of health maintenance and promotion facilities (“users”) as outlined below.

Personal information obtained

Health insurance code and number, name, gender, age, date of birth, relationship, address, telephone number, email address, and name and telephone number of place of employment

Purposes of use

To provide appropriate services to the users

To provide information needed by the users

To manage the operation of health promotion facilities (such as lodges) and administer subsidy payments appropriately

Before using the users’ personal information for any other purposes, we will provide the reasons for doing so to each user in question and obtain the user’s consent.

Provision of personal information to third parties

We will not provide the users’ personal information to external parties without the advance consent of each user in question.

4.

Subcontracting operations

Certain operations undertaken as part of these activities are subcontracted. Subcontractors are supervised regarding protection of personal information under subcontracting agreements. The main operations subcontracted are as follows:
Management of the operation of health promotion facilities (such as lodges), administration of subsidy payments, retention and disposal of documents.

Rights of the users
