How to apply for and undergo health examinations (for establishments)

Establishments	①	At the end of each fiscal year, establishments will be surveyed regarding whether they request health examinations to take place at the Examination Center on the second floor of Kenpokaikan. A detailed notice will be sent at a later date to the establishments that request health examinations at the Examination Center.
	②	Check the details and then submit applications collectively on an establishment basis.
	③	The Health Examination Information will be sent to the establishments based on the application.
	④	Distribute the Health Examination Information to the insured persons.
Insured persons	⑤	When you receive the Health Examination Information, make an appointment for the examination date via the Health Examination Appointment System (website) below.　* Health examinations are available by appointment only. Insured persons	⑥	Complete in advance the questionnaire form enclosed with the Health Examination Information and consent to “Handling of Personal Information” before undergoing the examination.
Establishments	⑦	Individual result reports and a result report for the establishment will be sent to the establishment. * Note that sending of result reports may be delayed due to reasons related to the process of preparing the reports. Additional applications are accepted at any time during the fiscal year. Fill out the Health Examination Addition (Change/Cancellation) Application Form and send it by fax or mail. * Applications for planned new employees should be submitted after obtaining Society eligibility.
