When an individual aged 65-74 is hospitalized at a convalescent facility, the patient must cover the standard personal cost burden for living expenses. The Health Insurance Society will pay amounts in excess of the standard personal cost burden as inpatient living expenses. The standard personal cost burden is 460 yen/meal up to three meals/day for meal expenses and 370 yen/day for accommodation expenses.

However, as with long-term care, insurance measures are available to reduce the personal cost burdens of low-income earners, depending on their income status. In addition, the burden corresponding to meal expenses is reduced for patients who remain in high need of hospitalization, such as patients suffering from an intractable disease or spinal cord injury, and for patients hospitalized in rehabilitation facilities.



Standard personal cost burden for living expenses

Meal expenses: these correspond to food costs and preparation costs

460 yen per meal

Accommodation expenses: these correspond to utility charges

370 yen per day

Some medical institutions require payment of 420 yen per meal for meal expenses, depending on the system they use for providing meals.

The standard personal cost burden for living expenses is the same for insured persons and their dependents. The system for covering high-cost medical care benefits does not apply to this personal cost burden.

Benefits for living expenses of hospitalized dependents are paid as dependents' care expenses.

For patients with designated intractable diseases, the copayment for meal expenses is 260 yen, while the copayment for accommodation expenses is 0 yen.


Measures to reduce personal cost burdens depending on income status

Persons with low income less than 70 years of age *1
Persons with low income II *2

Those with low degrees of medical necessity (Medical Care Category I)

Meal expenses: 210 yen per meal
Accommodation expenses:
370 yen per day

Those with high degrees of medical necessity (Medical Care Categories II or III)

Meal expenses: 210 yen/meal
Accommodation expenses: 370 yen/day (0 yen for patients with designated intractable diseases)

Low-income I *3 Those with low degrees of medical necessity (Medical Care Category I)

Meal expenses: 130 yen per meal
Accommodation expenses:
370 yen per day

Those with high degrees of medical necessity (Medical Care Categories II or III)

Meal expenses: 100 yen/meal
Accommodation expenses: 370 yen/day (0 yen for patients with designated intractable diseases)

Ex.: Persons with low income refer to insured persons and dependents exempted from municipal tax or to insured persons and dependents who do not require public assistance if they are approved as eligible for measures for reducing cost burdens for persons with low income.

Ex.: Persons aged 70-74 who are insured persons and dependents, and exempt from paying municipal tax

*3:

Ex.: Persons aged 70 or above in households whose insured persons and dependents are all exempt from paying municipal tax and whose incomes meet certain standards (e.g., annual pension income of no more than 800,000 yen)


Measures to reduce personal cost burdens depending on degree of medical condition and content of treatment

Patients hospitalized in rehabilitation facilities

These patients are required to pay only the burden corresponding to meal expenses, at the same amount as Inpatient meal standard expenses.

? What are medical treatment facilities?

Hospitalization is categorized under the Medical Service Law. The categories are general care, medical treatment care, care for tuberculosis, care for infectious diseases, and psychiatric care.
While care for tuberculosis, care for infectious diseases, and psychiatric care apply only to specific types of patients, general care and medical treatment care apply to all patients in general. General care is provided primarily for patients with acute conditions, while medical treatment care is provided primarily for patients with chronic conditions.
