Those insured with the Health Insurance Society
Eligible persons may undergo health examinations provided as part of the Society’s health activities no more than once during the same fiscal year.
Those who plan to undergo or have already undergone the health examinations under @-B below are not eligible. Those who undergo more than one health examination will be billed for the entire cost of all subsequent examinations.

iQj Upper digestive tract (stomach) examination
Insured persons aged 35 and older (not including those who have undergone or plan to undergo a complete medical checkup during the same fiscal year) |

Examination items as stipulated in Article 44 of the Ordinance on Industrial Safety and Health or examination items conforming to the courses recommended by the Health Insurance Society.
However, hepatitis viral examination and colorectal cancer screening are not eligible. |

Those aged 34 years and younger (not including those aged 30 years): Examination A (may also undergo Examination B)
Paid in the amount of actual health examination costs per person minus 2,000 yen, not to exceed 3,000 yen. |

Those aged 30 years and those aged 35 and older: Examination B
Paid in the amount of actual health examination costs per person minus 4,000 yen, not to exceed 6,000 yen. |

Those aged 40 years and older at the ages specified by the Health Insurance Society for Examination S (ages 40, 43, 45, 48, and 50, and at the same intervals after age 50): Examination S
Paid in the amount of actual health examination costs per person minus 5,000 yen, not to exceed 10,000 yen (only when the examination included items corresponding to Examination S). |
iQjUpper digestive tract (stomach) examination
X-ray (using barium) or endoscopy (gastric camera)
Paid in the amount of actual health examination costs, not to exceed 7,000 yen. |

Note that examinations treated as insured medical consultations and treatment are not covered. |
iRjGynecological examinations
Paid in the amount of actual health examination costs for breast cancer screening (multiple tests, such as palpation, ultrasound, and mammography, are available) and cervical cancer screening (multiple tests, such as internal examination, cervical cytology, and ultrasound, are available) combined (not including tumor marker testing for either of the above screenings), not to exceed 7,000 yen in total. |

Note that examinations treated as insured medical consultations and treatment are not covered. |

Apply at once for both screenings if you got breast cancer and cervical cancer screenings at different medical care institutions or if you got these screenings on different dates and therefore received multiple receipts.
E Subsidy applications are accepted only once during the same fiscal year.
E You may also choose to apply for just breast cancer or cervical cancer screening, instead of both screenings.

iSj Reexaminations, detailed examinations, and follow-up examinations
These are treated as insured medical consultations and treatment. |

4. How to apply for and get an examination |

The entire establishment (branch or office) applies directly to the preferred contracted medical care institution after obtaining consent to the Health Insurance Society’s policies on the Handling of Personal Information. |

When applying, provide information such as the examinees’ health insurance codes and numbers, names, genders, and dates of birth. |

The examinees undergo the examination after the applying establishment and the medical care institution have made scheduling and other arrangements. |

After payment to the examining medical care institutions is complete, each branch or office submits the following documents to the Health Insurance Society (Health Management Department) for each examining medical care institution: |

The Health Insurance Society will pay the subsidy after reviewing and making a decision on the application. Costs treated as those of insured medical consultations and treatment are not covered. Note that costs remaining after payment of the difference in amounts at the contracted medical care institution also are not eligible. |

The application must be submitted during the same fiscal year as the examination. |
