Family health examinations, upper digestive tract (stomach) examination, and gynecological examinations for dependents

 

Since December 2, 2024, health insurance eligibility is verified using one of the following:

@ Health insurance card
A Eligibility Verification Certificate
B Eligibility information screen on Mynaportal
C Myna health insurance card and Notice of Eligibility Information
D Online verification of eligibility using the Individual Number Card (only at facilities that have adopted the relevant system)


Family health examinations
 

Those who wish to get family health examinations can undergo them in the following four ways:
1.Health examination at the examination center on the second floor of Kenpokaikan
2.Health examination at a contracted medical care institution
3.Health examination at another medical care institution
4.Specific health checkup only at a group contracted facility (only those aged 40 and older)



1. Eligible persons

Dependents aged 18 and older
Eligible family members may undergo family health examinations (specific health checkups) only once per fiscal year (April 1–March 31). Note that those who already have undergone a family health examination, complete medical checkup, or specific health checkup through the Society’s health activities will need to pay the entire cost of these examinations.



1.Family health examination at the examination center on the second floor of Kenpokaikan (3-1-2 Nihonbashi Kayabacho, Chuo-ku, Tokyo) * Available by appointment only
 

2. Examination period

 

May through March of the following year
See the health examination schedule. Examinations are provided on separate dates for male and female examinees.
Health examination reception hours: 9:20 – 11:45 am (mornings only)
Cashier reception hours on the fifth floor: 9:20–11:40 am

3. Examination items

 

See the “Examination B” column on the list of examination items.

* For those aged 40 and older, the examination items of a specific health checkup are also included.

4. Examination costs

 

2,000 yen: Pay at the reception counter of the Health Management Department on the fifth floor of Kenpokaikan on the day of the examination.

* Reexaminations, detailed examinations, and follow-up examinations are provided free of charge (However, reexaminations, detailed examinations, and similar examinations for hearing, vision, funduscopy, and colorectal cancer screening are treated as insured medical consultations and treatment.)

5. How to apply for and get an examination

 

(1)

Apply by completing the necessary information on the Family Health Examination Application and sending it to the Health Insurance Society (Health Management Department) by post or fax.

(2)

Based on the application, the Health Insurance Society will send a questionnaire form and other documents to your home address. After receiving the questionnaire form, go to the health examination appointment website below to make an appointment for an examination date.

(3)

Consent to the Health Insurance Society’s policies on the Handling of Personal Information, enter the necessary information on the questionnaire form sent to you in advance and go directly to the Kenpokaikan on the examination date. Before doing so, stop by the Health Management Department on the fifth floor to pay the examination cost of 2,000 yen.

(4)

Examination results will be sent to your home address around the Friday two weeks after the examination date.



2.Family health examination at a contracted medical care institution
 

2. Examination period

 

April 1 through March 31 of the following year

3. Contracted medical care institutions

See the list of contracted medical care institutions.
You are eligible to use medical care institutions for which health examination cost is indicated in the “Family health examinations” column.

4. Examination items

See the “Examination B” column on the list of examination items.

* For those aged 40 and older, the examination items of a specific health checkup are also included.

5. Examination costs

Pay at the contracted medical care institution the portion of the contracted amount beyond the subsidy amount of 7,000 yen. You may not apply for the subsidy for the copayment amount remaining after paying to the contracted medical care institution the costs remaining after subtracting the subsidy amount.

* At a medical care institutions for which double circle is indicated in the "Family health reexaminations" column, reexaminations, detailed examinations, and follow-up examinations are provided free of charge in principle. At other facilities, they are treated as insured medical consultations and treatment. (Detailed examinations for colorectal cancer screening and similar examinations are treated as insured medical consultations and treatment.)

6. How to apply for and get an examination

(1)

Apply directly to your desired contracted medical care institution for an examination. Inform the institution that you are applying for a family health examination under the Tokyo Securities Industry Health Insurance Society and provide information such as your health insurance code and number, name, gender, and date of birth. You do not need to notify the Health Insurance Society after this. Consent to and sign the Health Insurance Society’s document Handling of Personal Information and submit to the medical care institution before the examination. (Consent to Handling of Personal Information is required to undergo an examination.)

(2)

Settle the above costs on the day of the examination.

Only those aged 40 and older

(3)

If you are eligible for specific health guidance (e.g., motivational support, proactive support) based on the examination results, the Health Insurance Society will send you a notice of specific health guidance.

(4)

Bring documentation verifying your health insurance eligibility with you to the providing facility to receive specific health guidance.



3.Family health examination at another medical care institution
 

You may apply for subsidy reimbursement after the examination.


2. Examination period

 

April 1 through March 31 of the following year

3. Examination items

Undergo the examination items that conform to “Examination B” on the list of examination items.
However, hepatitis viral examination and colorectal cancer screening are not eligible.

4. Examination costs

Pay the specified amount to the medical care institution directly.
You can apply for subsidy reimbursement (up to 7,000 yen) at a later date.

* Reexaminations, detailed examinations, and follow-up examinations are treated as insured medical consultations and treatment.

5. How to apply for and get an examination

(1)

Choose your desired medical care institution, consent to the Health Insurance Society’s policies on the Handling of Personal Information, and apply to the institution directly. You do not need to notify the Health Insurance Society after this.

(2)

Pay all examination costs to the medical care institution on the date of the examination. Be sure to obtain a receipt.

5. Applying for the subsidy

(1)

After you have received the examination results, etc., submit the following documents to the Health Insurance Society (Health Management Department):

 

Subsidization Application (for individual applications)

Receipt (original) (clearly indicating a description of the charge such as “Health examination charge,” the examinee’s name, and the date of the examination)

Copy of the notice of health examination results (copy of the page showing the date of the examination, the examinee’s name, and the results)

If the examinee is aged 40 or older, the standard questionnaire or equivalent questionnaire

(2)

The Health Insurance Society will pay the subsidy after reviewing and making a decision on the application.

(3)

Note that applications are accepted only during the fiscal year of the examination.

The subsidy may be paid through bank transfer. If you request payment by bank transfer, payment may be made only to the account in the name of the insured person.

Available for persons aged 40 and older only

(4)

If you are eligible for specific health guidance (motivational support, proactive support) based on the examination results, the Health Insurance Society will send you a notice of specific health guidance.

(5)

Bring documentation verifying your health insurance eligibility with you to the providing facility to receive specific health guidance.



4.Specific health checkup only at a group contracted facility (for persons aged 40-74 years only)
 

* A checkup ticket sent from the Health Insurance Society is required.


1. Eligible persons

Dependents aged 40-74 years who wish to get a specific health checkup only

2. Examination period

June 1 through March 31 of the following year

3. NFHIS group contracted facility

Refer to the National Federation of Health Insurance Societies website.

4. Examination items

See the “Specific health checkup” column on the list of examination items.

5. Examination costs

Free

6. How to apply for and get an examination

(1)

Check the name, expiration date, and other information on the checkup ticket sent from the Health Insurance Society.

(2)

Choose from the list of NFHIS group contracted facilities to get the examination and make an appointment directly. Be sure to inform the facility that you would like to undergo a specific health checkup.

(3)

On the examination date, bring your checkup ticket and documentation verifying your health insurance eligibility to the examination.

(4)

If you are eligible for specific health guidance (motivational support, proactive support) based on the examination results, the Health Insurance Society will send you a notice of specific health guidance. Bring documentation verifying your health insurance eligibility with you to the providing facility to receive specific health guidance. (No copayment applies.)




Upper digestive tract (stomach) examination
 

Those who wish to get an upper digestive tract (stomach) examination can take one of the following three ways:
1.Examination at Kenpokaikan and nearby contracted stomach examination facility
2.Examination at a contracted medical care institution
3.Examination at another medical care institution



1. Eligible persons

Dependents aged 35 and older
However, since stomach examinations are provided only once during the fiscal year (April 1–March 31), those who have already undergone a stomach examination or complete medical checkup as part of the Health Insurance Society’s health management activities are not eligible.



1.Examination at Kenpokaikan and a nearby contracted stomach examination facility

2. Examination period

 

May 1 through March 31 of the following year

* If you are getting a family health examination at the examination center on the second floor of Kenpokaikan and a stomach examination at a nearby contracted stomach examination facility on the same date, undergo the family health examination at Kenpokaikan first.

3. Examination items

 

X-ray (using barium) or endoscopy (gastric camera)

4. Examination costs

 

2,000 yen: Pay at the examining institution on the date of the examination.

* Reexaminations, detailed examinations, and follow-up examinations are provided free of charge.

5. Contracted facilities

 

Contracted stomach examination facilities refer to the following nine facilities.

6. How to apply for and get an examination

 

(1)

Provide the necessary information on the family health examination application and affix your seal, then send to the Health Management Department by post or fax.

* The application form is the same one used for family health examinations at the examination center on the second floor of Kenpokaikan.

(2)

Information on the above facilities and the appointment slip/questionnaire will be mailed based on the application.

(3)

Choose your desired facility and make an appointment directly by telephone.

(4)

On the examination date, be sure to consent to the Society’s “Handling of Personal Information” and bring @ documentation verifying your health insurance eligibility and A the appointment ticket/medical questionnaire with you to the examining medical care institution. You cannot get examined if you do not provide your consent or if you fail to bring documentation verifying your health insurance eligibility and the appointment ticket/medical questionnaire with you.

(5)

Pay the examination cost of 2,000 yen at the examining medical care institution, then receive your examination.



2.Examination at a contracted medical care institution
 

2. Examination period

 

April 1 through March 31 of the following year

3. Contracted medical care institutions

 

See the list of contracted medical care institutions.
You can use medical care institutions for which a double circle is indicated under the “Upper digestive tract (stomach) examination” column in the above list or a single circle is indicated in the list of contracted medical care institutions in the pamphlet “Health Checkup Information.”

4. Examination items

 

X-ray (using barium) or endoscopy (gastric camera)

* Some contracted medical care institutions provide only one or the other of the above examination items.

5. Examination costs

 

Pay at the contracted medical care institution the portion of the examination costs beyond the subsidy amount of 7,000 yen. Check with the contracted medical care institution directly for examination costs. You may not apply for the subsidy for the copayment amount remaining after paying to the contracted medical care institution the costs remaining after subtracting the subsidy amount.

* At a contracted medical care institution contracted to provide reexaminations as well, reexaminations, detailed examinations, and follow-up examinations are provided free of charge in principle.

6. How to apply for and get an examination

 

(1)

Apply for an examination directly to your desired contracted medical care institution. Inform the institution that you are a Tokyo Securities Industry Health Insurance Society dependent and provide information such as your health insurance code and number, name, gender, and date of birth. You do not need to notify the Health Insurance Society after this.
Consent to and sign the Health Insurance Society’s document Handling of Personal Information and submit to the medical care institution before the examination. (Consent to Handling of Personal Information is required to undergo an examination.)

(2)

Following the contracted medical care institution’s instructions, @ pay the above costs after the examination. If the medical care institution does not offer the option of paying only the difference from the subsidy amount, A be sure to obtain a receipt and then apply for the subsidy in the same manner as described under “3. Examination at another medical care institution.”



3.Examination at another medical care institution
 

You can apply for subsidy reimbursement after the examination.


2. Examination period

 

April 1 through March 31 of the following year

3. Examination items

 

X-ray (using barium) or endoscopy (gastric camera)

4. Examination costs

 

Pay all examination costs to the medical care institution on the date of the examination.
You can apply for subsidy reimbursement (up to 7,000 yen) at a later date.

Note that costs treated as those of insured medical consultations and treatment and costs remaining after payment of the difference in amounts at a contracted medical care institution are not eligible for subsidy reimbursement.




Gynecological examinations
 

Those who wish to get gynecological examinations can take one of the following two ways:
1.Examination at a contracted medical care institution
2.Examination at another medical care institution



1. Eligible persons

(Female) dependents aged 20 and older
Eligible female dependents may undergo gynecological examinations only once per fiscal year (April 1–March 31).
Note that if you get the same examinations multiple times, you will no longer be eligible for the subsidy and must bear the entire cost of the examinations.

2. Subsidy amount

7,000 yen (total for both breast cancer screening and cervical cancer screening)

3. Examination period

April 1 through March 31 of the following year

4. Examination items

Breast cancer screening (Multiple tests, such as palpation, ultrasound, and mammography, are available.)
Cervical cancer screening (Multiple tests, such as internal examination, cervical cytology, and ultrasound, are available.)

* Breast cancer screening must include an ultrasound or breast X-ray (mammography).

* Cervical cancer screening must include a physician-collected cervical cytology and internal examination. A transvaginal ultrasound also is eligible for the subsidy.

* Excludes tumor marker testing for breast or cervical cancer.



1.Examination at a contracted medical care institution
 

5. Contracted medical care institutions

 

See the list of contracted medical care institutions.
You can use medical care institutions for which a single circle and “(payment of difference in amounts accepted)” is indicated in the “Gynecological examinations” column.

* While gynecological examinations are generally conducted at the same time as other examinations such as family health examinations or complete medical checkups, at a medical care institution for which a single circle is indicated in the “Individual examinations available” column you may undergo gynecological examinations alone.

6. Examination costs

 

Pay at the contracted medical care institution the portion of the examination costs beyond the subsidy amount of 7,000 yen. Check with the contracted medical care institution directly for examination costs.

Reexaminations, detailed examinations, and follow-up examinations are treated as insured medical consultations and treatment.

7. How to apply for and get an examination

 

(1)

Apply for an examination directly to your desired contracted medical care institution.
Inform the institution that you are a Tokyo Securities Industry Health Insurance Society dependent and provide information such as your health insurance code and number, name, gender, and date of birth. You do not need to notify the Health Insurance Society after this. Consent to and sign the Health Insurance Society’s document Handling of Personal Information and submit to the medical care institution before the examination. (Consent to Handling of Personal Information is required to undergo an examination.)

(2)

Get the examination as instructed by the contracted medical care institution. Pay the portion of the costs beyond 7,000 yen on the date of the examination.
Note that if you got breast cancer and cervical cancer screenings at a medical care institution which does not offer the option of paying only the difference from the subsidy amount or if you got these screenings at different medical institutions, pay the full amount of the costs up front, then apply for the subsidy in the same way as described in “2. Examination at another medical care institution.”
Apply at once if you got the screenings at multiple medical care institutions.
・ Subsidy applications for gynecological examinations are accepted only once during the same fiscal year.
・ You may also choose to apply for just breast cancer or cervical cancer screening, instead of both screenings.



2.Examination at another medical care institution
 

5. Examination costs

 

Pay all examination costs directly to the medical care institution on the date of the examination. You can apply for subsidy reimbursement (up to 7,000 yen total for both breast cancer and cervical cancer screenings) at a later date.

Reexaminations, detailed examinations, and follow-up examinations are treated as insured medical consultations and treatment.

6. How to apply for and get an examination

 

(1)

Choose your desired medical care institution, consent to the Health Insurance Society’s policies on the Handling of Personal Information, and apply to the institution directly. You do not need to notify the Health Insurance Society after this.

(2)

Pay all examination costs to the medical care institution on the date of the examination. Be sure to obtain a receipt.

7. Applying for the subsidy

 

(1)

After you have received the examination results, etc., submit the following documents to the Health Insurance Society (Health Management Department):

 

Subsidization Application (for individual applications)

Receipt (original) (clearly indicating a description of the charge such as “Breast cancer/cervical cancer screening charge”)

Copy of the notice of examination results (copy of the page showing the date of the examination, the examinee’s name, and the results)

The Health Insurance Society will pay the subsidy after reviewing and making a decision on the application.

(3)

Note that the application must be submitted by the end of the fiscal year in which the examination was administered.

The subsidy may be paid through bank transfer. If you request payment by bank transfer, payment may be made only to an account in the name of the insured person.

The subsidy for gynecological examinations may be paid no more than once per fiscal year. You may apply for the subsidy for both breast cancer screening and cervical cancer screening or for just one of these. You may apply for the subsidy only once per fiscal year even if each of these screenings was administered on a separate date or at different medical care institutions. Be sure to apply once for both screenings together in such a case.

Note that costs treated as those of insured medical consultations and treatment and costs remaining after payment of the difference in amounts at a contracted medical care institution are not eligible for subsidy reimbursement.


Other information
Some municipalities provide (gynecological) cancer screening roughly once every two years. Check with your municipality and take advantage of such screening if available.

* Copayments incurred for such screening also are eligible for the subsidy.
